Medical Information

Medical Care at Camp Smile-A-Mile SAM combines the fun of camp with expert medical care for each child. Campers' medications, in their original containers, are turned in at The Health Hut and are distributed by the camp medical staff. The medical personnel are from the Hematology/Oncology Department at Children's of Alabama. They are available at all times of the day or night to provide chemotherapy and specialized care.

Each child attending camp, whether it is family camp, sibling camp or weeklong camp is required to submit a medical form. On therapy patients must have their oncologist fill out, review and sign their medical form. Off therapy patients may have their pediatrician sign the medical form. Sibling medical forms must also be signed by their pediatrician. Parents must fill out their own medical form when attending family camp. A doctor signature is not required for parent forms. 

To fill out the patient and/or sibling medical form and parent medical forms, please click this link to register for Camp SAM this year and fill out the basic medical information and print a copy of the form the doctor will need to sign for patients and siblings.
